We make every effort to provide value for your dental care. Billing and collection procedures are time consuming and costly. To help control you cost, we require that you pay your estimated portion at the time services are provided. Your services, or those of your dependents, are your financial responsibility. We accept cash, checks, debit cards, VISA, MasterCard, Discover, American Express, and CareCredit and LendingClub (both of which are health care payment plans that allow you to pay for your services over time). For those who qualify, CareCredit and LendingClub offers short term interest free plans or an extended payment plans at a low interest rate.
For your convenience, we will extend the courtesy of filing your primary insurance claim. Additional coverage is your responsibility. We are a Delta Dental Premier network provider as well as having many patients with a wide range of dental benefit plans. Please realize we do not work for, nor are we representatives of, any insurance company and may not be able to answer all your insurance questions. Our benefits estimates cannot be guaranteed to be fully accurate. Please familiarize yourself with your individual plan, including deductible, maximum payable benefits, and any exclusions. You are financially responsible for your services not paid for by insurance. As is common in the healthcare industry, we do require the SSN of the patient age 18 and older and policy holder/responsible party as well as copies of valid Driver's Licenses to file any claim and accept assignment of benefits. If you do not wish to provide all these forms of information, you may simply pay for your services and file your claim personally. We respect your privacy and protect all information with strict legal protocols and up-to-date secure technology.
